Tribute Gifts
Tribute gifts are a meaningful way to make a donation in the name of friends or family members
They may be "in celebration" donations, including birthdays, anniversaries, weddings or any special occasion. Tribute gifts also include gifts that or "in memory" donations, which honour the deceased and their family with a lasting and meaningful contribution to support Carleton Place & District Memorial Hospital (CPDMH). Sometimes tribute gifts are made “in honour” of loved ones or that are made in appreciation of care received at the Carleton Place and District Memorial Hospital.
When we receive a tribute gift, we send a special card to the person or the family of the person being honoured. The amount of the gift is kept confidential.

To make a donation by cheque, please download a Tribute Gift Form (requires adobe reader).
When sending your tribute donation by mail, please address the envelope to:

For example, if the donation is being directed to a specific piece of equipment or department, the envelope should be addressed to:

Please make all cheques payable to:
“The Carleton Place and District Memorial Hospital Foundation” or “CPDMH Foundation”
On the memo line of the cheque, please write the equipment or department name (if applicable) and the name of the person you are honouring with the donation.
For example, if you are donating $20 to the Diagnostic Imaging Department in honour of John Smith, the cheque would look like this:
